alcuni parametri base e su specifici questionari che, raccolti regolarmente dall’utente a
domicilio, possono dare
un’allerta tempestiva sul peggioramento di malattie croniche già diagnosticate e
seguite negli stessi centri specialistici che prescriveranno la app “MetaClinic-home”
Le malattie croniche che sono monitorizzabili dalla app “MetaClinic-home” sono: Diabete,
Broncopneumopatia Cronica Ostruttiva e Scompenso Cardiaco.
Questo sistema non è in grado di gestire le emergenze cliniche.
L’applicazione ha l’obiettivo di evidenziare, tra le visite programmate ai centri
specialistici, sintomi e dati clinici rilevabili dal paziente stesso che possano far
presupporre un peggioramento della malattia. Lo scopo è quello di mettere a
disposizione dell’operatore sanitario dati e questionari
secondo una scadenza prestabilita dallo specialista stesso per ogni soggetto. Sintomi
che troppe volte vengono ignorati dai pazienti, che sarebbero di facile
individuazione e che, se individuati opportunamente, potrebbero evitare
riacutizzazioni delle stesse malattie croniche.
Il sistema permette anche scambi di messaggi tra gli operatori Sanitari ed il paziente
cronico che, a differenza delle usuali chat, vengono registrati
ed evidenziati sulla cartella Clinica Smart Digital Clinic di METEDA in uso presso gli
stessi specialisti. Così l’operatore sanitario ed il medico specialista
potranno integrare le informazioni che vengono dal domicilio alla storia clinica del
paziente stesso nella sua interezza. In questo modo gli operatori possono decidere,
in maniera più oculata e precisa, il tipo di comunicazione o richiesta da fare rispetto
al successivo controllo ambulatoriale del paziente che si sottopone al monitoraggio.
L’attivazione dell’applicazione sul telefono del paziente avviene su decisione dello
specialista della malattia cronica da monitorare e solamente in sua presenza,
evitando così problemi di identificazione della persona da controllare.
