Popis
CheckWare Go gjør det mulig for deg å foreta enkel selvrapportering om egen helse. I tillegg kan den kobles opp mot sensorer via Bluetooth. Da kan man lese av f.eks. vekt, blodtrykksmåler og pulsoksymeter og sende disse dataene gjennom CheckWare Go inn til CheckWare-løsningen. Fra CheckWare-løsningen kan man koble mot elektronisk pasientjournal (EPJ). Hvilke sensorer som kan brukes må spesifiseres gjennom avtaler mellom CheckWare og våre kunder.
Data som samles inn vurderes av helsepersonell. Disse har beslutningsstøtte i CheckWare-løsningen. Behandleren har tilgang til kliniske rapporter som viser både dagens status og historisk utvikling. Både du og din behandler kan bli varslet om de individuelle terskelverdiene passeres, eller det er tegn til forverring. I tillegg gis det varslinger dersom løsningen observerer at rapporteringen fra deg uteblir. Dette trygger behandling i eget hjem.
Observasjoner, varslinger og kliniske beslutningsstøtterapporter kan gis direkte i CheckWare-løsningen eller via integrasjoner til andre systemer. Helsepersonell kan etablere digital kontakt med deg via sikre meldinger eller video.
Appen kan inkludere instruksjoner og illustrasjoner for hvordan sensor-målingene utføres. Dette bidrar til å gjøre målingene mer intuitive for deg. Umiddelbar tilbakemelding på målingene kan fremvises i brukergrensesnittet i appen.

Uansett hvor pasientene befinner seg, kan de sende et tilpasset skjema med helseoppdateringer til behandler. Behandleren får umiddelbart tilgang til kliniske rapporter som viser både dagens status og historisk utvikling.
