Description
De forma histórica y al ser enfermedades multiorgánicas, el fenotipo de cada EAS viene determinado por una combinación de distintos órganos afectados, con patrones de frecuencia y asociación que difieren de una enfermedad a otra. Tras la sospecha clínica, la aplicación de técnicas diagnósticas específicas, junto con la detección de alteraciones analíticas e inmunológicas, completan el workup de las EAS.
La heterogeneidad de las EAS originó desde sus primeras descripciones la necesidad de disponer de un listado de las principales manifestaciones de cada EAS que facilite la sospecha diagnóstica. Son criterios establecidos mediante un consenso de expertos internacionales en cada EAS para que las reglas diagnósticas sean las mismas allá donde se apliquen. Así nacieron los criterios inicialmente bautizados como “diagnósticos” y que en la actualidad se consideran “clasificatorios”. Los criterios clasificatorios permiten identificar con una elevada sensibilidad y especificidad (habitualmente superiores al 90%) el diagnóstico de una EAS concreta. Al no alcanzar el 100% se prefiere evitar el término “criterios diagnósticos” ya que no cubren las presentaciones clínicas más inhabituales de las EAS.
Es un placer presentar esta aplicación que permite evaluar de forma rápida y sencilla el cumplimiento de los criterios clasificatorios de las principales EAS, un proyecto desarrollado por wikiGEAS® con el aval del grupo de trabajo de EAS (GEAS) de la Sociedad Española de Medicina Interna (SEMI) y de la Universidad de Barcelona. No es una aplicación de confirmación del diagnóstico, que deben realizarlo los especialistas en EAS, y tampoco contiene todos los detalles y matices de las respectivas publicaciones, por lo que siempre recomendamos su lectura. Su principal valor y utilidad es aplicar una primera valoración de cumplimiento de criterios que ayude a identificar cuál es la principal EAS de sospecha.
